Standard Formulation Details
Pheniramine maleate for intramuscular injection is supplied in a ready-to-inject concentration and should be drawn directly from the ampule without any diluent. 1
The standard adult dose for acute allergic reactions and anaphylaxis is 10 mg IM (approximately 1 mL from the 2 mL ampule). 1
This translates to a concentration of approximately 22.75 mg/mL when the total content is 45.5 mg in 2 mL. 1
Pediatric Dosing from Standard Ampoule
For children > 12 years: 10 mg IM (≈1 mL) 1
For children 6–12 years: 5 mg IM (≈0.5 mL) 1
For children 6 months–6 years: 2.5 mg IM (≈0.25 mL) 1
For infants < 6 months: 250 µg/kg IM 1
Critical Administration Points
No dilution is required for intramuscular use; dilution is only indicated when preparing a slow intravenous push (over 2–3 minutes). 1
For IV administration, the recommended dose is 1-2 mg/kg IV or IM, with a maximum initial dose of 50 mg for acute drug reactions. 1
IV administration should be given slowly over 2-3 minutes to avoid pain at the injection site and reduce the risk of seizures. 1
Important Safety Considerations
Epinephrine must be administered first in anaphylactic reactions; pheniramine is an adjunctive therapy and should be given after epinephrine and fluid resuscitation. 1
Pheniramine alleviates urticaria and pruritus but does not treat bronchospasm, laryngeal edema, or hypotension. 1
Respiratory depression may occur, especially when pheniramine is combined with other sedatives (e.g., benzodiazepines or opioids). 1
Continuous monitoring of vital signs and oxygen saturation is advised; be prepared to provide respiratory support if apnea develops. 1
