Sildenafil: Dosing, Contraindications, and Counseling
For erectile dysfunction, start sildenafil at 50 mg taken approximately 1 hour before sexual activity (maximum once daily), and titrate to 100 mg or down to 25 mg based on efficacy and tolerability; for pulmonary arterial hypertension, use 20 mg three times daily, with FDA-approved titration up to 80 mg three times daily if needed.
Erectile Dysfunction
Dosing
- Starting dose: 50 mg taken approximately 1 hour before sexual activity, no more than once daily 1
- Dose titration: Adjust to 100 mg for improved efficacy or reduce to 25 mg if side effects occur 1
- Special populations requiring 25 mg starting dose:
- Elderly patients (≥65 years)
- Hepatic or renal impairment
- Concurrent use of CYP3A4 inhibitors (ritonavir, saquinavir, ketoconazole, erythromycin, cimetidine) 2
Key counseling point: Most men ultimately require 50-100 mg for optimal efficacy 3. Starting at 100 mg has been shown to reduce anxiety about the next intercourse attempt (56% vs 39% with 50 mg) and improve treatment satisfaction without increasing adverse events 3.
Instructions to Maximize Efficacy
Patients must understand that sildenafil requires sexual stimulation to work—it does not cause automatic erections 1. Take on an empty stomach when possible, as high-fat meals can delay absorption. Allow adequate time for dose titration across multiple attempts before declaring treatment failure.
Pulmonary Arterial Hypertension
Dosing
- Standard dose: 20 mg orally three times daily (every 8 hours) 4, 5
- FDA-approved titration: May increase up to 80 mg three times daily if needed 6
- Recent evidence: The 2024 multicenter randomized trial demonstrated that 80 mg three times daily was noninferior to 5 mg for mortality and superior for time to clinical worsening and 6-minute walk distance 6. Based on this, the FDA revoked approval of the 5 mg dose and now allows titration to 80 mg three times daily 6.
Important distinction: Sildenafil for PAH (20 mg tablets) contains the same active ingredient as sildenafil citrate for ED (25/50/100 mg tablets). Patients must not take both formulations concurrently 5.
Absolute Contraindications
Nitrate Use (Most Critical)
Never prescribe sildenafil with any form of nitrates—this combination can cause life-threatening hypotension 5, 7. This includes:
- Nitroglycerin (tablets, patches, sprays, ointments)
- Isosorbide mononitrate or dinitrate
- Recreational "poppers" (amyl nitrate/nitrite) 5
The mechanism: Both sildenafil and nitrates increase cyclic GMP, causing profound vasodilation and potentially fatal blood pressure drops 7.
Other Contraindications
Warnings and Precautions
Cardiovascular Risk Assessment
ED itself is a risk marker for cardiovascular disease, as strong a predictor as smoking or family history of MI 1. Before prescribing sildenafil for ED, assess cardiovascular risk and communicate this to the patient and their primary care provider 1. Men with known or suspected coronary artery disease may benefit from exercise testing before resuming sexual activity with sildenafil 7.
Vision and Hearing Loss
- NAION (non-arteritic anterior ischemic optic neuropathy): Counsel patients to seek immediate medical attention for sudden vision loss in one or both eyes 5
- Sudden hearing loss: May occur with tinnitus and dizziness; requires prompt evaluation 5
Conditions Requiring Caution
Inform patients to discuss these conditions with their physician:
- Heart problems (angina, heart failure, arrhythmias, prior MI)
- High or low blood pressure
- Retinitis pigmentosa
- Penile anatomical deformities or Peyronie's disease
- Blood cell disorders (sickle cell anemia)
- Bleeding disorders or active peptic ulcer disease 5
Common Adverse Effects
The most frequent side effects are dose-related and generally mild to moderate 1:
- Headache
- Flushing (lowest with tadalafil among PDE5 inhibitors)
- Dyspepsia (lowest with avanafil)
- Nasal congestion
- Visual disturbances
- Dizziness
- Back pain
- Myalgia
These adverse events follow a dose-response pattern but rarely lead to discontinuation 1.
Special Populations
Diabetes and Post-Prostatectomy
Men with diabetes and those post-radical prostatectomy have more severe ED at baseline and respond less robustly to PDE5 inhibitors 1. These patients may require the maximum 100 mg dose and should be counseled about potentially reduced efficacy compared to the general ED population.
Drug Interactions
Reduce starting dose to 25 mg when used with strong CYP3A4 inhibitors, as these prolong sildenafil's half-life (normally 3-5 hours) 2, 7.
Critical Counseling Points
- Never combine with nitrates under any circumstances 5, 7
- For ED: Requires sexual stimulation; not an automatic erection pill 1
- For PAH: Do not substitute with ED formulations or combine with other PDE5 inhibitors 5
- Seek immediate care for vision/hearing changes 5
- ED diagnosis provides opportunity to screen for cardiovascular disease 1
- Efficacy maintained long-term (≥1 year) in clinical trials 2, 8
