Amantadine Dosing for Extrapyramidal Symptoms
For drug-induced extrapyramidal symptoms (EPS) in adults, start amantadine at 100 mg twice daily (total 200 mg/day), with the option to increase to 300 mg/day in divided doses if response is suboptimal. 11
Standard Adult Dosing
The FDA-approved dosing for drug-induced EPS is straightforward:
- Initial dose: 100 mg twice daily (200 mg total daily dose) 11
- Maximum dose: Up to 300 mg/day in divided doses for patients who don't achieve optimal response at 200 mg/day 11
- Onset of action: Typically within 48 hours 1
Critical Dose Adjustments
Elderly Patients (≥65 years)
Do not exceed 100 mg/day in patients 65 years or older due to age-related decline in renal function 232. This is a firm ceiling regardless of symptom severity, as elderly patients experience substantially higher rates of CNS side effects at higher doses.
Renal Impairment
Dose reduction is mandatory when creatinine clearance falls below 50 mL/min/1.73m²:
- CrCl 30-50 mL/min: 200 mg on day 1, then 100 mg daily 1
- CrCl 15-29 mL/min: 200 mg on day 1, then 100 mg every other day 1
- CrCl <15 mL/min: 200 mg every 7 days 1
- Hemodialysis: 200 mg every 7 days 1
Patients with Serious Medical Illness
Start at 100 mg once daily for patients with serious associated medical conditions or those receiving high doses of other antiparkinson drugs, then increase to 100 mg twice daily after 1-2 weeks if necessary 11.
Clinical Considerations
Monitor closely for CNS side effects (nervousness, anxiety, insomnia, difficulty concentrating, lightheadedness), which occur more frequently with amantadine than with anticholinergic alternatives like benztropine 22. If CNS effects develop with once-daily dosing, split the dose to twice daily 1.
Research evidence demonstrates that amantadine is equally effective as biperiden (an anticholinergic) for treating neuroleptic-induced EPS, with the advantage of potentially ameliorating mild tardive dyskinesia rather than exacerbating it 45. This makes amantadine particularly valuable when TD coexists with parkinsonian EPS.
Patients with seizure disorders require close observation as amantadine may increase seizure frequency 67.
Duration of Therapy
Continue amantadine as long as the offending antipsychotic is maintained. Some patients experience loss of effectiveness after several months; in these cases, either increase the dose to 300 mg/day or temporarily discontinue for several weeks before reinitiating 1. A pediatric case report demonstrated successful treatment over 3 months until complete symptom resolution 8.
Common Pitfall
The most critical error is failing to reduce the dose in elderly patients or those with renal impairment. The standard 200 mg/day dose that is safe in young adults with normal renal function becomes toxic in these populations due to drug accumulation.
