Calcium Gluconate Administration Using 50 mL D5W
When using 50 mL of D5W (5% dextrose in water), you need to proportionally adjust the calcium gluconate concentration based on the standard preparation protocol. The standard guideline recommends mixing 10 ampules (1000 mg or 10 mL each = 10 grams total calcium gluconate, containing 900 mg elemental calcium) into 1 liter of D5W or normal saline 1.
Concentration Calculation for 50 mL D5W
For a 50 mL bag of D5W, you would add:
- 0.5 ampules (500 mg) of calcium gluconate to achieve the same concentration as the standard 1L preparation
- This creates a concentration of approximately 10 mg calcium gluconate per mL (or 0.93 mg elemental calcium per mL)
However, this small volume is not practical for continuous infusion as recommended in guidelines. The 50 mL volume would be depleted too quickly at the standard infusion rate of 100 mL/hour 1.
Practical Recommendations
For bolus administration using 50 mL D5W:
- Dilute 1-2 ampules (100-200 mg calcium gluconate = 9.3-18.6 mg elemental calcium) in 50 mL D5W 2
- This creates a concentration of 20-40 mg/mL, which falls within the FDA-approved range of 10-50 mg/mL for bolus administration 2
- Administer over 10-20 minutes with cardiac monitoring 1
- Do NOT exceed 200 mg/minute infusion rate in adults or 100 mg/minute in pediatric patients 2
For continuous infusion:
- The 50 mL volume is insufficient; use at minimum 100 mL, preferably 500 mL to 1 liter 2
- For continuous infusion, the FDA-approved concentration range is 5.8-10 mg/mL 2
- Standard preparation: 10 ampules (10 grams calcium gluconate) in 1 liter D5W, infused at 100 mL/hour 1
Critical Safety Considerations
- Ensure secure IV access - preferably large bore needle to avoid extravasation and tissue necrosis 1
- Continuous cardiac monitoring during administration, as rapid infusion can cause cardiac arrest 1
- Do not mix with bicarbonate or phosphate - will form insoluble calcium salts 1
- Monitor serum calcium every 4-6 hours during intermittent infusions, every 1-4 hours during continuous infusion 2
- Each mL of 10% calcium gluconate contains 9.3 mg (0.465 mEq) elemental calcium 2
Monitoring Parameters
- Check ionized calcium or albumin-corrected serum calcium levels frequently 1
- Goal: restore calcium to just below normal reference range 1
- An IV dose of 15 mg/kg elemental calcium over 4-6 hours increases serum calcium by approximately 0.5-0.75 mmol/L 1
Bottom line: A 50 mL bag of D5W is too small for the standard continuous calcium infusion protocol. Use it only for diluting bolus doses (1-2 ampules in 50 mL), or preferably use a larger volume (500 mL to 1 L) for proper continuous infusion therapy.
