Standard Dosing by Indication
- Duodenal Ulcer: 20 mg once daily for 4 weeks 1
- Gastric Ulcer: 40 mg once daily for 4-8 weeks 1
- GERD/Symptomatic GERD: 20 mg once daily for up to 4 weeks 1
- Erosive Esophagitis: 20 mg once daily for 4-8 weeks 1

Important Considerations
The pharmacological effect of omeprazole lasts much longer than its 1-hour elimination half-life because it irreversibly inhibits the H+,K+-ATPase enzyme in parietal cells 4
For most indications, once-daily dosing is sufficient due to the prolonged acid-suppressive effect 5
Higher doses may be required in patients with severe acid hypersecretion; basal acid output (BAO) > 20 mmol/h typically requires higher doses 2
When treating H. pylori infection, higher potency PPIs (such as 40 mg rabeprazole or esomeprazole twice daily) are recommended over pantoprazole 3
