The protympanum is the anterior portion of the tympanic cavity (middle ear space) that serves as the final common pathway between the tympanic cavity and the external environment, connecting the middle ear to the Eustachian tube. 1

The protympanum represents a specific anatomical subdivision of the middle ear, positioned anteriorly within the tympanic cavity structure. 2
Spatial Position
- The protympanum is located anterior to the mesotympanum (central middle ear space), forming part of the comprehensive subdivision system that includes the epitympanum (attic) superiorly, hypotympanum inferiorly, and retrotympanum posteriorly. 2
- It extends from the anterior tympanic annulus toward the tympanic opening of the Eustachian tube, serving as the transitional zone between the middle ear proper and the Eustachian tube entrance. 3
Structural Development and Composition
- Almost the entire protympanum develops from different processes of the petrous bone, contrary to older anatomical descriptions. 4
- The tympanic bone contributes only to the posterior border of the lateral wall of the protympanum, with the petrous bone forming the majority of this structure. 4
Medial Wall Relationships
- The carotid canal containing the internal carotid artery (ICA) forms the main structure of the medial wall of the protympanum. 4
- In 26% of cases, the wall of the internal carotid artery itself forms the front wall of the adjacent hypotympanum, demonstrating the close anatomical relationship between the protympanum and critical vascular structures. 3
