Rexulti Starting Dose for Bipolar Disorder

Rexulti (brexpiprazole) is NOT FDA-approved for bipolar disorder and should not be used as standard treatment for this indication. 1

Current FDA-Approved Indications

According to the FDA label, Rexulti is approved only for:

  • Adjunctive treatment of major depressive disorder (MDD) in adults
  • Treatment of schizophrenia in adults and adolescents ≥13 years
  • Treatment of agitation associated with dementia due to Alzheimer's disease 1

Bipolar disorder is notably absent from this list.

Clinical Evidence Gap

While research has explored brexpiprazole for bipolar disorder, the results have been disappointing:

  • Two Phase 3 randomized controlled trials for acute bipolar mania (studies 080 and 081) both failed to meet their primary endpoints, showing no statistically significant difference between brexpiprazole 2-4 mg/day and placebo 2

  • A small pilot study (n=21) in bipolar depression showed some promise, but this was an open-label design without placebo control, making the results unreliable for clinical decision-making 3

  • One review noted that brexpiprazole "is not yet approved to treat bipolar disorder" and "deserves further efficacy studies" 4

Recommended Approach

For bipolar disorder treatment, use FDA-approved agents with established efficacy:

For Acute Mania:

  • First-line options: Lithium, valproate, aripiprazole, olanzapine, risperidone, quetiapine, or ziprasidone 5
  • Lithium is the only agent FDA-approved for bipolar disorder in adolescents ≥12 years 5

Key Clinical Caveat:

If a patient is already on brexpiprazole for a different indication (e.g., MDD) and develops bipolar symptoms, do not rely on brexpiprazole for mood stabilization. Add or switch to an evidence-based mood stabilizer or antimanic agent with proven efficacy.

The failed clinical trials suggest that brexpiprazole lacks sufficient antimanic efficacy despite being a dopamine partial agonist like aripiprazole (which IS approved for bipolar mania). This highlights that not all agents in the same pharmacological class have equivalent efficacy across indications.
