Sucralfate Dosing for Gastrointestinal Ulcers
For the treatment of active duodenal ulcers, sucralfate should be administered at a dose of 1 gram four times daily on an empty stomach for 4-8 weeks. 1
Dosing Regimens
Active Duodenal Ulcer Treatment
- Standard dosing: 1 gram four times daily on an empty stomach 1

Administration Guidelines
- Take on an empty stomach 1
- If antacids are prescribed for pain relief, they should not be taken within 30 minutes before or after sucralfate 1
- For the four-times-daily regimen: Take 30 minutes before meals and at bedtime 4
- For the twice-daily regimen: Take 30 minutes before breakfast and at bedtime 3
Efficacy and Considerations
- Sucralfate forms an ulcer-adherent complex at duodenal ulcer sites, protecting the ulcer and promoting healing 5
- Effective for NSAID-associated duodenal ulcers, particularly when the NSAID is discontinued 5
- Not effective for treatment or prevention of NSAID-related gastric ulcers 5
- Healing rates for duodenal ulcers with sucralfate (1g four times daily) are approximately 91.7% after 4 weeks of treatment 6
Important Limitations
- Sucralfate is not recommended for gastric ulcer prevention due to the availability of superior alternatives such as PPIs 5
- PPIs are the preferred agents for therapy and prophylaxis of NSAID and ASA-associated GI injury 5
Special Populations
- Elderly patients: Dose selection should be cautious, usually starting at the lower end of the dosing range due to potential decreased hepatic, renal, or cardiac function 1
Common Side Effects
- Constipation (occurs in approximately 2-4% of patients) 7, 4

While alternative dosing regimens like 2 grams twice daily have shown similar efficacy to the standard four-times-daily regimen in clinical studies, the FDA-approved dosing remains 1 gram four times daily for active duodenal ulcers and 1 gram twice daily for maintenance therapy.
